I have read Craftibility’s volunteer liability and EEO and Harassment policies.

I, on behalf of myself, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Craftibility Foundation from any or all claims, actions or losses for bodily injury, property damage, loss of services or otherwise which may arise out of participation in the Inclusion Club activities.  I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have presently or in the future. 

I HAVE READ THE ABOVE WAIVERS AND RELEASE AND BY SIGNING IT AGREE.  IT IS MY INTENTION TO RELIEVE THE CRAFTIBILITY FOUNDATIONFROM LIABILITY FOR PERSONAL INJURY, DEATH, AND PROPERTY DAMAGE.
