Craftibility Publicity Waiver and Image License
Craftibility Foundation, a Missouri Not-for-Profit Organization (“Craftibility”) desires to use and publicize my name, image, and/or likeness (“I” or “Me”) for advertising, promotional, and other marketing purposes. I hereby authorize and consent to Craftibility to make these desired uses in accordance with the terms set forth in this Publicity Waiver and Image License (“Agreement”). I hereby irrevocably authorize, grant, license, and permit Craftibility the right to display, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, and modify my name, image, and likeness into materials in any medium or format that are created by or on behalf of Craftibility for advertising or promotional purposes without further consent from me. To the extent I have provided an image of myself for use with the foregoing, I further irrevocably grant a non-exclusive, royalty-free license to such image, and represent and warrant that I have the necessary rights to grant such a license in the provided image.
I further represent and warrant to Craftibility that I have the full right, power, and authority to enter into this Agreement and grant the rights thereafter, that I will provide only true and correct statements in connection with this Agreement and Craftibility’s use of the image, and the rights and licenses granted hereunder do not violate or conflict with any contract, agreement, or commitment entered into with a third party including, without limitation, my employer.
