WAIVER/REGISTRATION FOR MIXED-MEDIA CLASSES
Course Information and Waiver
Craftibility Foundation based in St. Louis, MO provides crafts classes children up to ages 4 years through 12 years old. The cost of the program should be paid via Paypal, Zelle, check, or cash.
The classes focus on arts and crafts projects. During these activities, your child and his belongings may be exposed to the possibility of injury, loss, and damage based on your child’s participation in the class. We have, however, taken precautions to reduce risk and provide a safe, educational, and inclusive environment. You acknowledge and understand these risks, and agree that you will not hold Craftibility Foundation responsible for any personal injury or property damage as a result of your child’s participation in the program.
During the program, we may take pictures and video of your child engaged in various program activities, as well as the artwork created by your child. Your child will have artwork to keep at home after every class for your family to enjoy. You agree that we may use these pictures and videos with or without modification in relation to this program, including promotion of Craftibility in a wide variety of mediums, without further compensating for such usage. The pictures and video may be made available to you upon request. The pictures and video will not otherwise be used without your permission, and they will never be sold
